New Patients

So you're new to Apex...well, we hope you won't be able to say that for long! At Apex Physical Therapy, we're anxious to get to know you, and excited to begin helping you improve your life through movement.

We're committed to making things easy, so below we've organized the information that you'll need to know before your first visit.  We're glad you've made it this far, and we look forward to meeting you soon!

Your First Visit

Your first visit to Apex Physical Therapy will begin with introductions to our staff, and will include a thorough evaluation of your current status.  We'll work with you to establish a plan of care, and often we'll initiate the plan during the first visit. 

We'll collect some information from you (forms available below), collect your initial payment or co-pay, and will schedule a portion of your remaining physical therapy sessions at the same time.

Following your visit, your physical therapist will document your session and will communicate with your physician as necessary.  Your physical therapist will then be available to you during your follow up treatment sessions, or by phone if required.

Forms

For your convenience, all intake forms are available for download.  By downloading and completing the forms prior to arriving at Apex Physical Therapy, you can expedite your first session, and allow your physical therapist the time necessary to review your information even before you leave the house!

Payment

At Apex Physical Therapy, our service to you is our top priority.  In order to ensure that we can provide the highest quality of care possible, it is important for us to collect payments for service in a timely manner. 

For your reference, we've made our insurance plan information, financial policy, and payment methods available to you online.  We require that co-payments are made at the time of service, and offer a number of payment options for your convenience.

Finding Us

Apex Physical Therapy is located in downtown Madras at 230 SW 5th St.  For your convenience, we've provided you with a map to our location.  Disabled parking is available.
